Important

Select the State Where You Will Be Located During Your Evaluation

State licensing requirements apply to telehealth evaluations. Your appointment may be assigned or moved to a provider who is licensed in the state where you are physically located at the time of the evaluation.

Before You Book

Why We Ask for Your State

Mental health providers must comply with professional licensing requirements for the location where the client is physically present during a telehealth evaluation.

Providing your state allows Valor Health to route your request to an appropriately licensed provider and helps protect the validity and credibility of the resulting medical documentation.

  • Helps match you with an appropriately licensed provider
  • Supports compliance with state telehealth requirements
  • Reduces the chance your appointment must be reassigned later
  • Helps ensure the resulting medical evidence is professionally valid

Professional Standards

Licensing Matters in Telehealth

The goal is not simply to schedule an appointment. It is to connect you with a provider who can appropriately conduct the evaluation from the state where you are located.

Your Location May Affect Your Appointment

If your physical location changes before your appointment, notify Valor Health as soon as possible.

A provider who can evaluate you in one state may not be authorized to conduct the same telehealth evaluation when you are physically present in another state.

In some situations, changing locations may require Valor Health to assign a different provider or adjust the appointment.
